The undersigned, being either (i) the individual whose likeness, voice, or statements may appear in the media content described below; or (ii) the legal owner of, or having full legal rights or authorization to permit commercial use of, media content of the property located at the address identified below, hereby grants to ProVia LLC, its affiliates, and advertising and public relations contractors (collectively “ProVia”), the irrevocable right and permission to use images, video, audio, or testimonials (“Media Content”) as specified below. This Media Content may feature me personally, my likeness, and/or my property, including the residence or other structures that utilize ProVia products, whether such Media Content was submitted by me to ProVia or produced solely by ProVia. This consent includes the right to use, reuse, publish, and assign, in any manner at all, said Media Content, in whole or in part, modified or altered, either by themselves or in conjunction with other imagery, video, or audio in any medium or form of distribution, and for any purposes whatsoever, including without limitation all promotional and advertising uses, non-commercial or commercial display, broadcast, exhibition of the final production, and other trade purposes, as well as using my name in connection therewith, in any format, including but not limited to print, digital, web, and social media.

By signing below, I acknowledge and agree that:

• All Media Content, for which consent is given, shall constitute ProVia’s sole property, and that ProVia has the full right to dispose of such Media Content in any manner whatsoever.
• ProVia may use, edit, reproduce, distribute, copyright and publish the Media Content at its sole discretion.
• I waive the right to inspect or approve the finished product or copy of any Media Content, including any use thereof.
• I hereby forever release and discharge ProVia and its respective representatives, employees, affiliates, and assignees from any and all claims, actions and demands arising out of or in connection with the use of said Media Content, including, but not limited to, any and all claims for invasion of privacy and libel.
• I acknowledge that this consent and authorization was signed by me willingly, and I represent and warrant that I am (i) the individual whose likeness, voice, or statements may appear in the Media Content, or (ii) the homeowner or authorized legal representative of the homeowner of the depicted property. I further represent and warrant that I have full authority and capacity to grant the authorizations set forth above, including rights relating to my personal likeness, voice, statements, and/or the depicted property.
